Welcome to a New Level of Care
Your Quality Care Coordinator will call you for your introduction to Chronic Care Management (CCM). They will review your current conditions, active problems, and areas of care that need more attention. You and your Quality Care Coordinator will determine how often you may need a CCM call.
You will be assigned a team (Blue, Purple, or Green) and that team will consist of a Quality Care Coordinator, a Navigator Care Coordinator, a Medical Provider, and a Nurse.
Care When and Where You Need It
Patients choose Quality Healthcare Services when they need a high level of individualized care. We provide options in terms of where you receive your care, offering home and in-person office visits. This concierge level of care is billable to your insurance or can be paid by you.
We also partner with a mobile X-ray company and a mobile lab draw company to provide home-based services for homebound clients. If you have a concern that needs to be addressed, you do NOT need to wait until your next scheduled CCM call. You will receive your Quality Care Coordinator's direct extension so you may contact them when you wish.

Appointments
Your Care Coordinator will manage your scheduled appointments with your physician who will follow up with you accordingly and as needed. Routine appointments are determined by your physician, but if you feel like you need a sooner appointment please call your Care Coordinator so we can schedule accordingly.
If you are enrolled in one of our Physician Outreach Programs and we are unable to send a physician out within the time frame needed to assess your concerns, you still have access to your Primary Care Office.
FAQs
Quality Healthcare Services provides a much higher level of care that goes beyond basic home health. In addition to home health and in-office care, you will have a dedicated team working with you to address your medical care, including a Quality Care Coordinator. Your Quality Care Coordinator provides Quality Care Management via telephone and assists with record retrieval, referral coordination, medication refills, and also addresses your general questions and concerns.
Patients are referred to Quality Healthcare Services due to their complex medical conditions, high-risk status, need for specialist care, recent hospitalizations and/or recent ER visits. Patients include post-surgery, homebound, hospice, chronic care patients, and other patients that need a higher level of care than most practices can provide.
Providers conduct home visits on Tuesdays, Thursday, and Weekends.
Currently, all of our services are covered by Classic Medicare (Medicare part B) as long as they are medically necessary for the patient. For all other insurances, please contact the office for more information.
Additional services can be provided upon request and charged out-of-pocket. We will always be transparent with you if you a service is not covered by your insurance.
In addition to consultations and checkups, we can perform skin biopsies and joint injections in the office for your convenience. Our patient beds raise and lower to accommodate patients who have difficulty getting on fixed-height exam beds.
